How to Apply to Heritage Yeshiva Seminars

1. Complete the Application form in its entirety.

2. NAME - Your PASSPORT legal name ONLY.

3. MEDICAL INFORMATION- Are you allergic to penicillin, sulfa drugs etc.?

Does the Heritage physician need to know something "medical" about you?
If not - write "none".

4. Are you a Vegetarian? Yes or No.

5. ROOMMATE PREFERENCE- you and 1 other person. If your roommate is from another school, indicate the school next to the roommate's name.

6. Read the statements regarding Heritage responsibilities.

7. Indicate whether you are registering for POLAND ONLY or  POLAND AND PRAGUE.

8. Complete the CANCELLATION STATEMENT and have your parent(s) affix a signature.

9. DEADLINE FOR APPLICATIONS AND NON-REFUNDABLE DEPOSITS - DECEMBER 15. Late fees of $75 must accompany registration after this date.

10. FINAL PAYMENTS DEADLINE - JANUARY 15. Late fees of $75 must accompany registration after this date.

11. Forward the Application along with a non-refundable BANK CHECK of $300 payable to Heritage Seminars to:
